What Is Myofascial Pain Syndrome? Understanding the Hidden Source of Muscle Pain

If you’ve ever experienced a persistent, nagging muscle ache that doesn’t seem to go away — even after resting — the problem may not simply be “tight muscles.”
Myofascial pain syndrome (MPS) is a common source of chronic or recurring muscular pain. It involves the muscles and the surrounding connective tissue, known as fascia, and can produce pain that is localised to one area or referred to another part of the body.
Understanding the relationship between muscle and fascia can help explain why some aches and pains persist despite stretching, rest or conventional massage.
What exactly is myofascial pain?
The term myofascial combines two words:
Myo — relating to muscle
Fascial — relating to fascia, the connective tissue that surrounds and connects muscles, bones, nerves and other structures.
Myofascial pain syndrome is generally characterised by painful areas within muscle and fascia, often associated with myofascial trigger points.
A trigger point is a sensitive area within a muscle that can feel like a small, taut or irritable band of tissue. Applying pressure to the area may reproduce the familiar pain you have been experiencing — and sometimes the pain can be felt somewhere else entirely.
This is known as referred pain.
For example, a trigger point in the upper trapezius muscle may contribute to pain around the neck, shoulder or even the side of the head. Similarly, trigger points in muscles around the hip can sometimes contribute to pain that is perceived around the lower back or buttock.
Why does myofascial pain develop?
There isn't always one single cause.
Myofascial pain can develop following repetitive movements, sustained postures, overuse, physical injury or changes in how the body is loading and moving.
For people who train regularly, repetitive loading without adequate recovery can contribute to muscular sensitivity and altered movement patterns.
For others, the issue may come from something much less obvious — such as spending long periods sitting at a desk, driving, working in one position or repeatedly performing the same movement at work.
Stress and poor sleep may also influence how the nervous system processes pain and how effectively the body recovers from physical stress.
Common contributing factors include:
Repetitive movements and overuse
Poor or sustained postures
Previous injuries
Sudden increases in training volume
Inadequate recovery
Prolonged sitting or inactivity
Muscular weakness or altered movement patterns
Physical and psychological stress
Often, it's not one particular event that creates the problem. Instead, small amounts of stress can accumulate over time until the tissues become increasingly sensitive.
What does myofascial pain feel like?
Myofascial pain can present differently from person to person.
Common symptoms include:
A deep, aching muscle pain
Localised tenderness
Stiffness or restricted movement
Muscles that feel tight or “knotted”
Pain that returns repeatedly in the same area
Pain that is reproduced when specific points are pressed
Referred pain into another area of the body

One of the frustrating aspects of myofascial pain is that the place you feel pain isn't necessarily where the problem originates.
This is why simply treating the painful spot doesn't always produce lasting results.
The importance of fascia
Fascia is a continuous connective tissue network throughout the body. Rather than thinking of muscles as completely separate structures, it can be more useful to understand them as part of an interconnected system.
Changes in one region can influence movement and load elsewhere.
For example, restrictions or altered muscle function around the hip may influence how the pelvis moves, which can subsequently affect the lower back. Similarly, restrictions around the shoulder and thoracic spine may influence how the neck
and shoulder muscles work.
This doesn't mean that every pain problem is caused by fascia. Rather, the interaction between muscles, fascia, joints and the nervous system is an important part of understanding musculoskeletal pain.
Why does stretching sometimes not fix it?
Stretching can be useful, but it isn't always enough.
If a muscle continually feels tight, there may be more going on than simply a lack of flexibility.
The nervous system may be increasing muscular tone as a protective response. A joint may not be moving efficiently. Another muscle may be compensating for weakness or poor motor control. Or repetitive loading may continue to irritate the tissues.
In these situations, repeatedly stretching the same muscle may provide temporary relief without addressing the underlying contributors.

How is myofascial pain treated?
Treatment should be individualised according to the person, their symptoms and the factors contributing to their pain.

Your pain is more than just a “knot”
One of the biggest misconceptions about persistent muscular pain is that there is simply a “knot” that needs to be rubbed out.
Pain is considerably more complex than that.
Muscles, fascia, joints and the nervous system constantly communicate with one another. When an area becomes overloaded or sensitised, the body can alter how it moves and distributes load.
This can create a cycle:
Overload → increased sensitivity → altered movement → further overload → persistent pain.
Breaking that cycle often requires more than simply treating the area that hurts.
When should you seek professional help?
If muscle pain is persistent, repeatedly returning, limiting your movement or interfering with exercise, work or everyday activities, it may be worth having it properly assessed.
A qualified practitioner can look beyond the painful area and consider how you move, how you load your body and whether other areas may be contributing to the problem.
It is also important to remember that not all pain is myofascial. Persistent, severe or unexplained pain should be appropriately assessed, particularly when accompanied by symptoms such as significant weakness, numbness, unexplained weight loss, fever, trauma or other concerning changes.

Myofascial pain syndrome isn't simply about having “tight muscles.”
It is a complex pain condition involving muscles, fascia and the nervous system, often associated with sensitive areas within muscle and referred pain.
Understanding why an area hurts is often just as important as treating the pain itself.
